Location: Complete Care at Margate Park, Chicago, IL
The Registered Nurse (RN)/ Licensed Practical Nurse (LPN) is responsible for providing direct nursing care, supervising nursing assistants, and leading the Restorative Nursing Program (RNP). This dual role requires adherence to all state and federal guidelines, focusing on maximizing resident independence and maintaining the highest degree of quality care.

The Registered Nurse (RN)/ Licensed Practical Nurse (LPN) is responsible for providing direct nursing care, supervising nursing assistants, and leading the Restorative Nursing Program (RNP). This dual role requires adherence to all state and federal guidelines, focusing on maximizing resident independence and maintaining the highest degree of quality care.
Essential Duties And Responsibilities
Restorative Nursing Program (RNP) Leadership
- Program Management: Plan, develop, implement, evaluate, and direct the facility's comprehensive RNP to prevent functional decline and maximize the resident’s existing abilities.
- Interdisciplinary Coordination: Serve as the primary liaison between Nursing and Therapy to ensure the effective transition of residents from skilled rehabilitation to a maintenance RNP.
- Assessment & Care Planning: Conduct functional observations and participate in MDS/Care Plan development, ensuring all restorative measures are accurately reflected, documented, and evaluated quarterly or as needed.
- Staff Oversight: Monitor and supervise Restorative Nursing Assistants (RNAs) and staff. Provide training and ensure competency in proper restorative techniques (e.g., ambulation, range of motion, ADL independence).
- Resource Management: Ensure all necessary adaptive equipment (splints, assistive devices) is obtained, available, functional, and properly utilized by residents.
- Clinical Care: Prepare and administer medications, and perform professional services (e.g., IV care, catheterization, wound care) as ordered by the physician.
- Supervision: Direct the day-to-day functions of the nursing assistants, providing leadership and planning shift services, programs, and activities.
- Documentation & Reporting: Complete all required recordkeeping upon resident admission, transfer, or discharge. Transcribe physician orders, chart nurse's notes descriptively, and complete accident/incident reports.
- Monitoring: Conduct rounds with physicians as necessary. Monitor personnel to ensure adherence to established safety regulations.
- Compliance & Confidentiality: Perform all duties in accordance with local, state, and federal guidelines. Maintain confidentiality of all protected health information (PHI) and ensure resident privacy.
- License: Must possess a current, unencumbered, active license to practice as an RN or LPN in the state of Illinois.
- Certification: Current CPR Certification is required.
- Skills: Must be able to read, write, speak, and understand the English language, and make independent decisions when necessary.
- Knowledge: Must be knowledgeable of nursing, medical practices, and applicable laws/regulations pertaining to nursing care facilities.
- Experience in Long Term Care is preferred.
